Die Einwirkung von Hitze auf Gewebe führt zu einer zonenförmigen Schädigung. Dabei ist in der Koagulationszone der Zelltod irreversibel. Angrenzend sind in der sogenannten Stasezone die meisten Zellen initial noch vital. Wenn jedoch die Durchblutung in dieser Region nicht verbessert werden kann, kommt es zum Fortschreiten der Verbrennung, zum so genannten Nachbrennen. Eines der wesentlichen Punkte, um das Nachbrennen zu vermeiden, ist die Reduktion von Ödem und die Aufrechterhaltung der Mikrozirkulation. Wir stellen unsere ersten Erfahrungen mit der V.A.C.®-Therapie auf Verbrennungswunden zur Vermeidung des Nachbrennens vor. Während der letzten Monate wurden alle Patienten mit bilateralen Handverbrennungen in unser Therapie-Protokoll eingeschlossen. Jeweils eine Hand wurde mit V.A.C.® (KCI, Austria), die kontralaterale Hand mit Flammazine (Smith & Nephew, Germany) behandelt. Die ersten Resultate zeigen, dass beide wichtigen Faktoren (Ödem und Mikrozirkulation) positiv von der V.A.C.® Therapie beeinflusst werden. Thermal injury applied to living tissue results in zones of injury. Cell death is complete in the zone of coagulation. Beneath this area, there is the zone of lesser injury, where most of the cells are initially viable. If this zone of stasis is not reversed, the burn wound will progress. One of the major aspects to prevent progression is to reduce the edema formation and to preserve microcirculation. We present our first experiences to prevent the progression by use of topical negative pressure. Within the last months, all patients with bilateral partial thickness hand burns were included into this treatment protocol. Within one patient, one hand was treated with the V.A.C.® (KCI, Austria), the contra lateral one by use of Flammazine (Smith & Nephew, Germany). Our first observations and data indicate, that both important factors (edema and microcirculation) could be influenced positively by use of the V.A.C.®
