Abstract
To the Editor: In a letter to the Journal (July 15, 1982, issued), Kwok et al.1 reported on retinal cotton-wool spots caused by Pneumocystis carinii in a male homosexual with P. carinii pneumonia. Their report was followed by a number of letters that questioned and even expressed doubts about their findings. Holland et al.2 had "grave reservations concerning this apparent association," and Sobel3 stated that "their proof is not convincing."We studied a 32-year-old male homosexual with symptoms of fever, fatigue, left upper quadrant pain, lymphadenopathy, and splenomegaly of five months' duration who was hospitalized in October 1985 for splenectomy. . . .
