Two cases of pseudogynecomastia and their surgical treatment are described. This condition is usually bilateral and manifests itself as adiposis of the breast which may reach large proportions. Hormonal therapy rarely is effective; surgical reconstruction is the therapy of choice. These 2 patients were young [male][male] with breasts the size of those of a [female] of 14 yrs. of age. These [male][male] otherwise normally developed, displayed extreme sensitiveness about their condition and developed the mental attitude of a recluse. The surgical procedure in both cases consisted in a wedge-shaped excision of a segment of mammary tissue from the posterior aspect of the breast. The area of the nipple was left intact and the only incision made was concealed in the submammary fold. The microscopic examination in one case revealed it to be due to adipose tissue, and the other to be a fibrotic breast without significant changes in the epithelium. The postoperative improvement in the mentality of these 2 patients was remarkable and entirely out of proportion to the relatively minor surgical procedure.
