Its continued sporadic appearance, especially in the elderly, is explained as the reactivation, under certain conditions, of bacilli that have remained viable in dormant foci following infection earlier in life. The practical implication is that tuberculosis should be considered far more often in the diagnosis of chronic illness among older persons, whether or not they have recently been exposed to an active case.
