Fundoplication for Hypotensive Lower Esophageal Sphincter

Abstract
Anatomic correction of a hiatus hernia may fail to correct reflux in patients (like the one presented) who also have an incompetent LES; such patients should be identified preoperatively so that a sphincter-enhancing procedure can be incorporated into the hernia repair. The approach demonstrated is the Nissen fundoplication, in which the gastric fundus is used to fashion a collar that encircles the LES zone.
