Anti-A Autoantibody with Severe Intravascular Hemolysis

Abstract

Anti-A autoantibody manifested itself clinically in fatal hemolysis and kidney failure in the following case.Case ReportA 73-year-old man suddenly experienced severe bilateral lumbar pain associated with generalized weakness, jaundice and lethargy. He was hospitalized on the following morning, when he was semicomatose and grossly icteric and the spleen was palpable a few centimeters below the left costal margin. Neither purpura nor lymphadenopathy was noted, and the urine was free of bile. He was severely oliguric despite intravenous hydration and infusion of both 25 g of mannitol and 300 mg of hydrocortisone. A right plantar extension reflex was noted . . .
