Four patients are described who developed kernicterus. All infants were immature and with each patient there were multiple factors that may have favored the development of kernicterus. Serum bilirubin in all patients fell after phototherapy began, but phototherapy as used in these infants failed to prevent the occurrence of kernicterus.
