Painful ophthalmoplegia

Abstract
A syndrome of recurrent unilateral retro-orbital pain with extraocular palsies and occasional blindness is described. This syndrome is believed to be caused by a low-grade, nonspecific inflammation of the cavernous sinus. Careful history and physical examination, supported by negative angiography, are sufficient for making the diagnosis. Surgical exploration is unnecessary. Although spontaneous remission occurs, corticosteroid therapy probably is of value.
