Relief from pain in sudeck's posttraumatic syndrome by fasciotomy a clinical, radiographic, and scintigraphic study

Abstract
Twenty-three consecutive patients aged 33–80 years with a presumed Sudeck's syndrome of one hand or one foot were seen. A fracture initiated the syndrome in three-quarters of them, and the median duration of suffering was 3.5 months in the hand and 7 months in the foot. Osteoporosis and marked 99mTc-labeled methylene diphosphonate uptake were seen in radiographs and scintigrams respectively. Thirteen of the patients were operatively treated; distal fasciotomy on the volar aspect of the forearm or the ventral aspect of the lower leg gave rapid relief from pain at rest in nine of ten patients thus affected. All the patients became symptom-free, except two who underwent closed treatment. At follow-up 2–8 years later radiographic and scintigraphic findings were usually normal. 23 Patienten im Alter von 33 bis 80 Jahren wurden untersucht. Bei alien wurde ein Sudeck-Syndrom an einer Hand oder einem Fuß erwartet. In Dreiviertel der Fälle war eine Fraktur der auslösende Faktor. Die durchschnittliche Krankheitsdauer betrug bei den betroffenen Handen 3,5 Monate and bei den betroffenen Füßen 7 Monate. Röntgenologische and szintigraphische Untersuchungen ergaben eine Osteoporose and eine Anreicherung von 99mTc-markierten Methylen-diphosphonat in den betroffenen Händen and Füßen 13 von 23 Patienten sind operativ behandelt worden. Dabei wurde eine Fasziotomie an der Beugeseite des Unterarmes bzw. an der Vorderseite des Unterschenkels durchgeführt. Bei neun von zehn Patienten mit Ruheschmerzen trat eine rasche Linderung der Beschwerden ein. Außer zwei konservativ behandelten wurden alle Patienten beschwerdefrei. Bei der Nachuntersuchung nach 2–8 Jahren hatten sich die röntgenologischen and szintigraphischen Befunde normalisiert.This publication has 20 references indexed in Scilit:
