More than a decade ago, the Harvard Medical Practice Study^1,2 reported that complications of medical care, or adverse events (AEs), were responsible for considerable morbidity, mortality, and expense. More recently, the Institute of Medicine^3,4 issued highly publicized reports emphasizing the importance of systems design in reducing error.
