Electrocochleography with a Far Field Technique

Abstract
At the present time elctrocochleography is performed with many different techniques and the results differ accordingly. Far field volume—conducted potentials, with the electrodes placed at some distance from the signal source, appear to show such a degree of consistency that they offer possibilities for a precise interpretation of the different parts of the electrocochleographic response. The paper presents a systematic investigation of the far field potentials in normal subjects with the application of 4 kHz tonepips and a low frequency click. Active and reference electrodes were placed at each its mastoid and the ground at the forehead. Filter 0.5—4.5 kHz. Sweep time 10 msec. Repitition rate 10/sec. Against expectations the CM is clearly present down to 50—60 dB SL. Input—output functions and latencies for the different peaks are presented in curves. In a separate experiment the potentials were measured first at the mastoid homolateral to the stimulus and then at the contralateral mastoid. In the contra—lateral recordings the early part of the response corresponding to the classical N1 complex is missing while the later part is present with the same magnitude as homolaterally, but reversed in phase. It is concluded that these results not are consistent with the assumption that the neural part of the electrocochleographic response originates from one single signal source. Electrocochleografi er endnu på et experimentelt stade og den teknik der anvendes er ret forskellig. Det har vist sig at disse meget små elektriske svar kan registreres også i en vis afstand fra det indre øre som volumen—ledte fjernfelts potentialer. Rent teoretisk skulle man forvente en større ensartethed ved en sådan fjern—felts metodik og dermed have bedre mulighed for at tolke svarene. Artiklen prœsenterer en systematisk undersøgelse af disse potentialer hos normale. Som stimuli er brugt dels et 4 kHz tonepip og dels et lavfrekvent klik. Aktive og reference elektroder anbragtes på hver sin processus mastoideus, jordelektroden på panden. Imod forventning kunne man registrere cochleœr mikrofoni helt ned til 50—60 dB SL. Input—output funktioner for de forskellige komponenter er vist i kurve—form. I et sœrligt experiment måltes potentialerne først alene ved den processus mastoideus, som hører til det øre, hvor lydstimuli prœsenteres, og derefter ved den kontralaterale processus mastoideus. I det elektriske svar fra den kontralaterale side mangler det klassiske N1 komplex, mens de senere komponenter er til stede med samme størrelse som ved homolateral afledning, blot vendt med hensyn til fasen. Disse fund passer ikke med den antagelse at alle komponenterne i det elektrocochleografiske svar stammer fra den samme generator, og de støtter den teori at de senere komponenter beror på aktivitet i hjernestammen.Keywords
