Gastric Secretion before and after Graded Partial Gastrectomy for Duodenal Ulcer

Abstract
A gradation of gastrectomy for duodenal ulcer was performed in 80 patients according to the preoperative values of total HCl/h after large doses of histamine (MAO), In 44 patients subjected to two-thirds partial gastrectomy the preoperative average value of total HCl/h was 43.0 mEq. Postoperatively the outputs of total HCl/h following large doses of histamine were 4.7 and 5.0 mEq at three and twelve months respectively. A corresponding reduction was observed for the prehistamine values of total HCL-secretion, from 8.7 to 1.4 and 1.1 mEq/h respectively. In the group of patients subjected to one-third gastrectomy (36 patients) the average values of total HC1 were 21.8 mEq/h before operation, and 2.9 mEq/h at three months and 3.1 mEq/h at twelve months postoperatively. The prehistamine values were reduced from 4.9 to 1.1 and 1.2 mEq/h respectively. The results show that a satisfactory gradation of the extent of the partial gastrectomy for duodenal ulcer is possible by taking into account the preopeative values of total HCl/h following large doses of histamine.
