Transfusion‐Associated Fatalities: Review of Bureau of Biologies Reports 1976–1978

Abstract
Review of the 70 transfusion-associated fatalities reported to the Bureau of Biologics (BOB) between 1976 and 1978 revealed 44 acute hemolytic reactions, 2 delayed hemolytic reactions, 5 fatalities associated with acute respiratory failure, 2 cases of bacterial contamination, 1 graft-vs.-host reaction (GVHR), 10 cases of hepatitis (including some cases of hepatitis B infection) and 6 fatalities not associated with transfusion. Of 44 acute hemolytic reactions, 38 were due to ABO incompatibility. Clerical confusion was the cause of 33 of 37 cases in which error could be defined.This publication has 17 references indexed in Scilit:
