Orbital phlebography in the diagnosis of painful ophthalmoplegia

Abstract
Three cases of painful ophthalmoplegia have been described in which symptoms suggesting a tumor of the orbit justified neuroradiological assessment. Phlebography in each case revealed stenosis of the superior ophthalmic vein in its third portion, and non-opicification of the cavernous sinus. Hirtz incidences revealed contralateral cavernous sinus opacification and venous drainage through the coronary sinus. These neuroradiological findings helped to differentiate this syndrome from other affections which have similar signs and symptoms. Les auteurs rapportent trois cas d'ophtalmoplégie douloureuse dans lesquels l'importance des symptômes réalisait un syndrome de pseudo-tumeur de l'orbite justifiant des explorations neuro-radiologiques. La phlebographie a mis en évidence dans chacun des cas une sténose serrée de la veine ophtalmique dans sa troisième portion et l'absence d'opacification du sinus caverneux. Les incidences de Hirtz ont permis de visualiser le sinus caverneux contro-latéral et le drainage veineux par le sinus coronaire. Ces aspects neuroradiologiques permettent de différencier cette affection de celles dont la séméiologie est très voisine. Beschreibung von 3 Fällen einer schmerzhaften Ophthalmoplegie, bei denen der Verdacht auf einen Tumor bestand. Die Phlebographie zeigte jeweils eine Stenose der V. ophthalmica superior im 3.
